Using one index file in all directories and subdirectories
Is there a method to tell the webserver (Apache, nginx, ..) that if an index file is not present in a directory under document root, then it should use the specified index.php located elsewhere?

Redirect from index.html to its directory using .htaccess?
You can do this using mod_rewrite near the top of your .htaccess file. In its simplest form this would be something like:
