You can usually add whatever you need to add in an asynchronous way without slowing down the page.

We back our campaigns into a CPM and then give a fixed percentage of that to the publishers. So lets say we sell a campaign for $300 for 30 days. That's $10 a day. On a given day lets say they server 10k impressions, so that's $1/CPM. If a publisher served 1,000 of those ads they would get 60% of the $1. We have this all automated of course (in Adzerk). From a technology perspective it is fairly easy to recognize the same person clicking over and over again - and if they tied it back to your IP address you could definitely be banned.

Large brands like IE, Toyota, are focused on engagement these days on the web. They are less worried about a click-through and just want to make sure readers are paying attention to the ad, perhaps watching an embedded video, etc. I am sure they are looking at this as an engagement and .25-.50 is actually cheap as far as an engagement goes.

You could check out AdRoll - they are doing lots of retargeting lately which should help bump your revenue. It also doesn't rely on contextual ads as much as adsense. The other good option would be to find one in your vertical market - what is the site's focus?

I don't know of any ad network that will let you do that - mostly because people would game the system passing in fake keywords once they figure out what pays the best. There are ad servers you can use to do that - but then you would need to sell the ads yourself.
