Thanks for this comprehensive answer, thanks to you I was able to trace it to wp_cron.php which gives me some clear next steps.

comment Mixing self signed certs with traditional SSL
Thanks Paul. I think there was another answer here that was deleted--is that what you mean when referring to nginx? I wasn't sure what you meant by that. When I type in https://mydomain.com/wp-admin I am redirected to the subdomain the SSL is on (https://secure.mydomain.com).

comment Safely backup and migrate a hacked and infected WordPress site
No, this is for a site I was not the host for. It's the first host I ever had, shared hosting that constantly got hacked and was often down for months at a time. I've been continuing to pay $3/year for this hosting because I've been too lazy to clean the site up and migrate it to a new server with a new company. Now I'm doing it.

comment DNS records: make mydomain.com work without www
Well seeing as I freshly installed the OS on the server just a few minutes before doing all this, I have doubts that it's malware. I believe it is possible to get DNS to route to your website if the www record is not set, so I think that is what happened, and actually, now that I reset it, it is giving me that username/password requirement again.
