I have been googling for last few hours but no luck.

I am looking for a simple example on how I can query MySQL using PHP and then use the output in an input box.

My confusion is where do I put the PHP code. I added input box using <input> but I need to know where I add my PHP code and how does it access it.

Please share an example.
